Abstract
Theophylline (1, 3-dimethylxanthine), a number of the xanthine family was introduced for the treatment of asthma in 1900. Now it is well established as a highly effective bronchodilator for asthma and chronic obstructive lung disease. Theophylline has a short half-life and a narrow therapeutic index. So the rate of elimination of theophylline varies among patients in same dosage, that individual dose titration is desirable when given for long term therapy.
In recent years, sustained released preparation become developed in order to compensate these disadvantage. In theory, their twice-daily dosage regimen reduced patient compliance and fluctuation of blood level. But it is recommended to determination therapeutic dose by monitoring serum concentration in the initial phase of treatment. Methods of theophylline analysis are HPLC, turbidimetry, RIA, and EIA. In recent years, fluorescence polarization immunoassay (FDIA), a homogeneous competitive binding assay, is introduced. The TDXTM` micro processor-controlled fully automated FPIA using fluorophor as tracer, has been extremely favorable because it¢¥s acceptable precision, accuracy and stability. This once again support the need for minimizing manual methods in the stat environment, not only to rapidity but also accuracy.
The authors had a chance to evaluation Abbott TDXTM for stat drug monitoring, and perform quantitation of serum theophylline concentration in 24 patients who were given Ascontin (sustained released aminophylline) 450 mg-daily dose during 14/Feb. to 4/Apr.
1985.
So we obtained some results according to clinical evaluation in above 24 patients. The results are follow;
1) Serum theophylline concentration in therapeutic range (5.1-20.0¥ìg/ml) are 12(50%) among 24 patients.
2) 3 patients show toxic symptoms in therapeutic range. So toxic symptoms appears in this range that close clinical observation should be done during treatment.
3) 12 patients (50%) reveals below therapeutic range (0-5.pug/ml) and 8 patients (67%) are tuberculosis. We cannot confirm the interaction of disease itself and/or antituberculosis drugs in those patients. But it may important factor that the content of theophylline tablet in Korea is lower than in other countries.
